120P The compartment-specific spatial transcriptomic landscape of 3D cultured Gleason 7 prostate cancer

3D culture of patient-derived tumour tissue offers a route to personalized cancer treatment whereby potential drugs can be screened ex vivo stratify the optimal care plans. Such stratification predict patient responsiveness and adverse effects, with view streamlining first-line drug selection. Appropriate models need show biomimicry in biology origin, replicating relevant architecture, biomarker levels, cellular organisation pathways that allow accurate response testing drugs. 11 Gleason 7 prostate patients were recruited & their underwent MRI-guided bio-banking, following radical prostatectomy, using PEOPLE method. Matched specimens cultured as gelatin sponge explants for all 72 hours. Resulting samples segmented into epithelial vs. non-epithelial compartments via PanCK staining before undergoing whole genome spatial transcriptomics analysis. Spatial RNA expression analysis was performed total 17299 genes each patient. Epithelial reveals architectural maintenance core basal, luminal neuroendocrine epithelium post-culture. Non-epithelial showed immune landscape vascular coverage. However, 72-hour an increase key ECM degradation markers including MMPs ADAMTS family genes. Overall, reduction seen post-culture growth targets AR-response, WNT/Beta-catenin PI3K/AKT/mTOR apoptotic markers. This is first study date which provides in-depth tissue-compartmentally transcriptomic primary cancer, offering insight such clinical screening applications. while architecture preserved, drug-target involved cell viability less active culture, suggesting improvements are needed ensure responses mimic outcome matched patients.
